1. Visit www.philips.com.au/sleepservicespromotions 
2. Register and download the claim form from website
3. Complete all fields on the Claim Form with the Claimant's personal information
4. Email the completed Claim For to: sleepservices@philipspromotions.com.au together with a clear and legible scan or photo of the fully paid purcase receipt/tax invoice indicating store name, receipt number, date of purchase, model/items purchased, and amount paid for the Eligible Products
5. Submit all forms before the end of registration date: 5:00pm AEDT on 25 January 2021
